Inclusion Criteria
1. People living with T2DM for at least 1year and on oral medication/insulin/both
2. Residents of urban slums of Davangere for at least a year and will be available for next 2-3years
Exclusion Criteria
1. Bedridden/seriously ill/hospitalized
2. Non-consent
