Inclusion Criteria
- •Inclusion criteria
- •Type 2 diabetes
- •Years of disease: \>\= 6 months
- •Elevated hemoglobin A1C (\>7 %)
- •Having either mobile or home telephones
- •Exclusion criteria
- •patients who have a diagnosed psychotic disorder, disabling sensory impairment, or life expectancy of \<12 months, pregnant, active cardiovascular disease, end stage renal failure, breathing problems requiring hospitalization or oxygen use in the previous six months

- •Poor controlled diabetes
