Adult patients diagnosed with chronic heart failure who are in Stage C of the ACCF (American College of Cardiology Foundation)/AHA (American Heart Association)heart failure stage classification and meet the following selection criteria 1-3.
1.At least one hospital admission or discharge in the past year
2.One or more of the screening items apply(Poor adherence, such as living alone or with poor family functioning, comorbid mental problems (especially depression) or poor cognitive function)
3.Patients who are currently hospitalized at a medical institution where nurses with high expertise in the field of heart failure are affiliated, and who are scheduled to be transitioned to outpatient care at said medical institution by December 31, 2022.
The subject's own comprehension and judgment are impaired due to factors such as cognitive decline, making it difficult to obtain consent.
