Inclusion Criteria
18 years or older,
- CKD stages 1-4 for more than 3 months
Exclusion Criteria
- high health literacy (checklist/screener),
- major cognitive problems,
- too severely ill according the physician,
- life-threatening conditions (e.g. cancer)

Name Time Method Patients: eGFR, albuminuria, glycemia, BMI, blood pressure, support of healthcare professional, CKD knowledge, health literacy<br>Professionals: knowledge of health literacy, recommendations given by the healthcare professional<br>Cost-effectiveness: paid job, absence leaves, means of transportation and number of visits to the health service, help of significant others, number (and type) of medications used, hospitalization (ICU-infirmary) (number and length), other medical costs (imaging, laboratory tests, vaccines, pre-dialysis surgery)
