Inclusion Criteria

18 years or older,
- CKD stages 1-4 for more than 3 months

Exclusion Criteria

- high health literacy (checklist/screener),
- major cognitive problems,
- too severely ill according the physician,
- life-threatening conditions (e.g. cancer)
