Inclusion Criteria
All known Hypertensive patients registered with UHTC during the study period
Hypertensive patients with reading ability of English and Hindi (Care takers as proxy in cases of unable read)
Exclusion Criteria
Hypertensive patients unwilling to participate in the study.
Hypertensive patients with psychiatric illnesses.
Hypertensive patients with no reading ability of English and Hindi including care takers (proxy)
