Inclusion Criteria
1. Outpatients with ischemic stroke treated at the Stroke Clinic and the Neurology Clinic of Chulalongkorn Hospital within 1 year after discharge from ward of Chulalongkorn Hospital
2. Aged 18 years and above.
3. Patients can understand and communicate in Thai language
4. Willing to participate in the research.
Exclusion Criteria
1. Patients who have not received continuous treatment at Chulalongkorn Hospital.
2. Patients with cognitive impairment due to ischemic stroke.
