Inclusion Criteria

All inpatient (IPD) patients with diagnosis of heart failure confirmed by both primary physician and 2 major or 1 major and 2 minor Framingham criteria were included after informed consent

Exclusion Criteria

1) Pregnancy patients
2) Confused patients or disabled patients that cannot made their own decision
3) Patients that cannot be reached by phone call during follow up
