- •Inpatients of age > 18 years admitted to the medicine department
- •Patients with polypharmacy ( >5 prescription medications)
- •Patients suffering from one or more of the following conditions - Diabetes, Hypertension, Ischaemic Heart Disease, Asthma & COPD and Rheumatoid Arthritis
排除标准
- •cancer patients
- •organ transplant recipients
- •patients suffering from psychological problem(s)
- •those who were unable to participate in the assessm
