Eligibility Criteria
Inclusion Criteria
- •Inpatients of age \> 18 years admitted to the medicine department
- •Patients with polypharmacy ( \>5 prescription medications)
- •Patients suffering from one or more of the following conditions \- Diabetes, Hypertension, Ischaemic Heart Disease, Asthma \& COPD and Rheumatoid Arthritis
Exclusion Criteria
- •cancer patients
- •organ transplant recipients
- •patients suffering from psychological problem(s)
- •those who were unable to participate in the assessm
