入选标准
- •patient of inpatient geriatric rehabilitation;
- •member of AOK Baden-Württemberg as health insurance provider;
- •at least independent mobility at room level;
- •accessibility for intervention staff (residence < 30 km to the study center);
排除标准
- •terminal/fatal disease (life expectancy < 6 months);
- •institutional care after discharge from inpatient rehabilitation (long-term, respite, or short-term care);
- •outpatient/mobile rehabilitation following inpatient rehabilitation;
- •planned inpatient hospital stay in the first 12 weeks after discharge from inpatient rehabilitation;
- •moderate to severe cognitive impairment (Mini Mental Status Examination, MMSE < 21 points);
- •insufficient communication skills;
