Eligibility Criteria
Inclusion Criteria
- •1\. Persons diagnosed with HF for at least 6 months by the cardiologist.
- •2\. NYHA functional class II to III at the time of recruitment, which the cardiologist confirmed based on NYHA functional class criteria.
- •3\. Aged 40 years or older because of the risk of developing HF increases in this age (Benjamin et al., 2018; Virani et al., 2020\).
- •4\. Stable condition of comorbidities in terms of able to control blood pressure and blood sugar, no chronic kidney disease stage IV and V.
- •5\. No impaired vision or hearing.
- •6\. No cognitive impairment by screening with the Thai Mental State Examination (TMSE), the TMSE should be greater than 23 points.
- •7\. Has a mobile phone and can be contacted by LINE application.
- •8\. Able to read, write, and communicate in Thai language.
Exclusion Criteria
- •1\. The participants cannot continue in all sessions of intervention.
- •2\. The participants have severe symptoms or complications from heart or comorbidity during the period of the program.
- •3\. The participants have worsening conditions or transfer to the intensive care unit or death before the end of the program.
