简要总结
One of the commonest causes of difficult weaning has been diaphragmatic dysfunction, which progressively worsens while using mechanical ventilation. USG examination allows the direct visualization of the diaphragm thickness during active breathing & reflect the magnitude of diaphragmatic effort. Ultrasound is safe, non-invasive, without radiation hazards, and available at the bedside modality for assessment of lung and diaphragm for weaning of ICU patients. The study expects to evaluate diaphragmatic thickness fraction (DTF) assessed by ultrasonography as predictor during weaning from mechanical ventilation. This may provide a novel, non-invasive approach to predict successful extubation, potentially reducing reintubation rates and guiding personalized weaning strategies in critical care settings, thereby enhancing patient outcomes and optimizing resource utilization.

- •Age 18 to 65 years.
- •Mechanical ventilation for minimum 12 hours and maximum 7 days.
- •Intubated and mechanically ventilated patient ready to be weaned.
排除标准
- •Patients with history of neuromuscular disease Patients with history suggestive of diaphragmatic dysfunction Patients with chest tube in situ Pregnant Patients Tracheostomized patients.

时间窗: RSBI assessed 60 min after successful Spontaneous Breathing trial. | ABG done at 1 hour and 6 hapur after extubation.
