Inclusion Criteria

1. Patients >16 yrs upto 60 yrs who require AFOI for difficult airway.

2. MPG Classification III, IV

3. BMI > 30 Kg/m2

4. Mouth opening < 3 fingers

Exclusion Criteria

1. Age < 16 yrs

2. MPG Classification I,II

3. BMI <30 Kg/m2

4. Mouth opening > 3 fingers

5. Patients suffering from any hyperactive respiratory condition or known allergy to the taken premedications.

6. Patients with respiratory, hepatic, renal, neurological or psychiatric diseases

7. Patients with bradycardia (heart rate [HR] <60 beats/minute) or any type of Atrio - Ventricular block demonstrated in electrocardiogram (ECG), uncontrolled hypertension.

8. Patients who are pregnant or with any bleeding disorder with contraindication for nasal intubation, any known allergy to one of the study medications .

9. Patients with lack of cooperation or effective communication

10. Patients with abuse to any psychotropic drugs.
