1. Aged 18-65 years
2. Chronic non-specific low back pain for at least 6 weeks in the past 12 months
3. Provision of informed consent
1. Time in pain in last 12 months is <6 weeks
2. Surgery on the back less than 3 months ago
3. Injury to the back less than 3 months ago
4. Age below 18 years
5. Not on employer’s insurance plan
6. Refusal to provide consent
7. Indicate they did not experience low back pain when asked about the location(s) of pain (other options are upper back” and neck”)
