- •The person lives independenly (alone or with a partner)
- •Team 290 is the primary service provider to this client
- •a diagnosis related to dementia has been selected
Exclusion Criteria
- •diagnosis has been postulated before the age of 70 was reached
- •dementia is not the main diagnosis
