•Age group 20-40 yrs.
•Subjects having CBCT- periapical score 4 & 5. ( Diameter of periapical radiolucency >4-8 mm & >8mm respectively ) . This new PAI score was based on CBCT (CBCT-PAI) and introduced in 2008 by Estrela et al.[17]
•Failed re-RCT with persistent sign and symptoms of infection eg. Swelling, sinus, pus discharge etc.
•Subjects requiring periapical root end surgery involving anterior teeth.
•Subjects with sufficient motivation and capable of reporting back to the clinic for follow up visits.
•Subjects diagnosed with diabetes mellitus or any systemic diseases which is likely to influence healing
•Patients requiring re-apicoectomy.
•Smokers and alcoholics.
•Patients with bone disorders.
•Patients with anomalies of blood and immune system.
•Patients with malignant tumors and/ or on cancer therapy.
•Pregnant females.

Name Time Method Reduction in oedema, post operative pain, signs of infection, sinus and tenderness on percussion which will be evaluated clinically after 1 week post operatively <br/ ><br>Timepoint: 1 week after endodontic surgery
