• Adult patients ventilated more than 48 h

• Stable respiratory and hemodynamic conditions for SBT
• Consent of patients
• Arterial line

Exclusion Criteria

• Laryngeal dyspnea
• Tracheostomy
• Arythmya
• No echogenicity
• Paraplegia >T8
