To Study Bilateral Modified Thoracoabdominal Nerve Block Through The Perichondrial Approach Versus Port Site Infiltration For Post Operative Pain Relief After Laparoscopic Cholecystectomy.

Postoperative pain following laparoscopic cholecystectomy remains a significant clinical issue due to its multifactorial origin and potential to delay recovery if inadequately controlled. In accordance with Enhanced Recovery After Surgery (ERAS) principles, multimodal analgesia incorporating ultrasound-guided regional techniques is increasingly emphasized to enhance analgesia while minimizing opioid consumption. The modified thoracoabdominal nerve block through the perichondrial approach (M-TAPA) is an emerging regional analgesic technique that targets the thoracoabdominal nerves (T6–T12) by depositing local anaesthetic beneath the costal cartilage, potentially providing more effective and extensive analgesia than conventional transversus abdominis plane blocks. This randomized controlled study aims to compare the postoperative analgesic efficacy of bilateral ultrasound-guided M-TAPA block and port-site local anaesthetic infiltration in patients undergoing laparoscopic cholecystectomy under general anaesthesia. Patients will be randomly allocated into two groups: Group M will receive a bilateral ultrasound-guided M-TAPA block with 20 ml of 0.2% ropivacaine administered on each side, while Group P will receive 20 ml of 0.2% ropivacaine infiltrated equally across the four laparoscopic port sites. The results of this study are expected to clarify the relative effectiveness of these two analgesic approaches and support optimization of postoperative pain management with reduced opioid requirements following laparoscopic cholecystectomy.

  • Patients of either gender, age 18 years and above, American society of Anaesthesiologists(ASA)grade 1 and 2, undergoing laparoscopic cholecystectomy.

排除标准

  • Patients allergic to Ropivacaine, Infection at site of injection, Patients undergoing emergency surgery, patients with coagulopathies, refusal to participate in the study, patients unable to comprehend NRS score, Pregnant patients and patients with BMI more than 35.
