Inclusion criteria for stroke survivors
Male or female, aged 18 years or over.
survived a stroke for more than 6 months.
Willing to participate in the study.
Inclusion criteria for caregiver
Male or female, aged 18 years and over.
Had a family member who had survived from stroke for more than 6 months.
Able to communicate Thai or local Thai dialect.
Willing to participate in the research.
Inclusion criteria for community leader
Male or female, aged 18 years and over.
formal/ informal leader of community that had stroke survivor for more than 6 months.
Able to communicate in Thai or local Thai dialect.
Willing to participate in the research.
Exclusion criteria for stroke survivors
Had an acute illness that required hospitalization.
passed away before the data collection operation was complete.
Exclusion criteria for caregiver
Had an acute illness that requires hospitalization.
Unable to complete participation in the study
Exclusion criteria for community leader
Had an acute illness that required hospitalization.
Unable to participate in all duration of the study
