Inclusion Criteria

1. Age more than 18 years old and under 80 years old
2. Elective case for major GI surgery
3. Tracheal intubation and mechanical ventilation at least 12 hrs in ICU admission
4. Epidural Analgesia

Exclusion Criteria

1. Pregnancy and breast feeding
2. Organ or bone marrow Transplants
3.Delirium or dementia
4.Cerebrovascular accident
5. High grade atrioventricular blocks without cardiac pacemaker
6. Acute liver failure
7.contraindications for epidural analgesia
8. Allergy to propofol or dexmedetomidine
