Inclusion Criteria
Patient in a normal trauma surgery ward (no intensive care unit) with an indication for outpatient or inpatient rehabilitation, aged 18 and over. Communication possible (not demented or confused).
Exclusion Criteria
Patients in intensive care units or external wards, younger than 18 years of age, communication not possible (supervised, demented, confused, language barrier, etc.), isolated / infectious high-risk patients, patients who refuse a recommended rehabilitation measure.

- Primary Outcome Measures
Name Time Method The aim is to create a digital, patient-moderated, case-specific health record in which the patients themselves actively grant access rights to the follow-up therapists they have chosen.<br>The patient survey accompanying the study is intended to find out whether the patients are fundamentally willing to use such a system, to what extent the patients are technically equipped (smartphone, using websites etc.) and whether they have digital literacy.<br>The additional questioning of some test patients (who participate in the practical testing of the patkom-prototype) is intended to show whether the new system proves itself in clinical use and how the patients and other actors can cope with it.

Name Time Method The aim of the attached doctoral project is to determine whether patients are receptive to such a system in which they are actively involved or whether they act more conservatively and want to use conventional methods. In the second step, the experiences of a few test patients are going to be evaluated. Finally, expert interviews will be conducted with the actors involved in the acute and rehabilitation clinics in order to collect empirical values and to be able to assess the general benefit of such a project.
