1. Pre arthritic knee (posterior horn meniscal root tear with varus aligned knee greater than 5degrees)
2. Localized posteromedial joint line tenderness with confirmation of MMPRT on MRI
3. Varus aligned knee >5 degrees
4. KL grades 1,2,3 on a standing knee anteroposterior (AP) x-ray radiograph
1. Varus aligned knee more than10
2. KL grade greater than grade3
3. Patellofemoral arthritis
4. Cruciate/collateral ligament insufficiency
5. SPONK (Spontaneous osteonecrosis of the knee)
6. BMI more thAN 30

- Primary Outcome Measures
Name Time Method The clinical outcomes will be assessed in functional scores (Visual Analogue scale (VAS) scores, Knee injury Osteoarthritis Outcome Scores(KOOS), International knee documentation committee scores (IKDC)& Tegner Lysholm scores), radiological outcomes (Xray assessment -Hip-Knee-Ankle (HKA) Axis, Medial Proximal Tibial Angle(mPTA), Kellgren Lawrence (KL) grading. MRI assessment:Medial meniscus extrusion/Relative percentage of extrusion correction, healing of meniscus) for all the groups and second-look arthroscopy findings for operative groups.Timepoint: Patients will be followed up at 6 weeks, 3 months, 6 months, 1 year, 18 months, 24 months and yearly intervals then after.
- Secondary Outcome Measures
Name Time Method second look arthroscopy to assess loss of cartilage lesion grade 1 to 4 on ICRS scale, and meniscal healing as Complete healing, Lax healing, Scar tissue, Failed healing.Timepoint: A second look arthroscopy will be planned at 24 months.
