Eligibility Criteria
Inclusion Criteria
- •Patients on 5 or more prescribed drugs with at least one FRIDs
- •Willing to participate \& give consent
- •MoCA (Montreal Cognitive Assessment) with a score above 26 and can walk 10 meters or with a walking aid(walking stick, canes, callipers)
Exclusion Criteria
- •Critically ill patients,
- •Patients with known neurological illnesses, cognitive impairment/ Dementia, Parkinsonism, stroke
- •Elderly on homecare or special care
- •Older adults using walkers, wheelchair
