Study of deep neck space infections of patients presenting to Ent department Govt medical College, Cuddalore – Role of Ct and Ultrasound

Computerised tomography and Ultrasound at the time of presentation and within 5 days in the early disease and in determining the extent of the disease. Its use to help initiate early intervention with IV antibiotics and as guide for surgical drainage helps to successful management of such infections. Patients who present with symptoms indicative of Deep Neck infections are subject to Radiological investigation and those patients not responding to IV antibiotic therapy are subject to surgery

We aim to study the radiological features of Deep Neck space infections (DNI) and pattern of spread of infection and draw a correlation of duration of stay in hospital to number of deep neck spaces involved. Study aims to investigate the role of patient variables such as age, diabetes and level of neck infection to duration of stay and mortality. Prior studies have revealed the predictive value of mediastinal involvement to poor prognosis and role of early surgical intervention vs IV antibiotic therapy in reduced hospital stay and better prognosis. Future implications include role of Artificial intelligence as a predictor of patient prognosis with input of radiological data from stored dicom information. Many hospitals lack radiologist and specialist capable of dealing with complicated Deep neck infection and our study will provide valuable inputs to rural physician to seek early referral of such risk patients

入选标准

  • •All patients presenting to ENT dept with deep neck infections with symptoms of pain, swelling, difficulty swallow and difficulty breathing.
  • •Fasciitis and acute lymph node infections involving deep cervical nodes and presenting as neck swelling.

排除标准

  • •1.Neck infections as a result of Malignancy.
  • •2.Infections involving congenital Cysts and Fistulas.
  • •3.Patients refusing treatment and referred elsewhere are excluded.

时间窗: Early intervention with IV antibiotics and Insulin within 5 days and CT Scan and Ultrasound as a guide for surgical drainage helps to successful management of such infections. Patients who present with symptoms and those patients not responding to IV antibiotic therapy are subject to surgery. Treatment failure considered at end of 1 week, 2 weeks

次要结局

  • Involvement of multiple deep neck spaces has been attributed to poor prognosis and outcomes such as sepsis and death. Number of vertical and horizontal neck spaces is evaluated and patient outcomes are measured(2 weeks)
