Eligibility Criteria

Inclusion Criteria

  • •1\. Individuals diagnosed with diabetes and severe mental illness
  • •2\. Treated in one of the eight psychiatric outpatient clinics in the recruitment period
  • •3\. Aged \>\=18 years

Exclusion Criteria

  • •1\. Patients without diabetes
  • •2\. Patients not in the psychiatric outpatient clinics
  • •3\. Patients who do not wish to participate
