Inclusion Criteria
- •1\. Individuals diagnosed with diabetes and severe mental illness
- •2\. Treated in one of the eight psychiatric outpatient clinics in the recruitment period
- •3\. Aged \>\=18 years
Exclusion Criteria
- •1\. Patients without diabetes
- •2\. Patients not in the psychiatric outpatient clinics
- •3\. Patients who do not wish to participate
