1) Receiving at least one of the selected drugs in the study with the most inappropriate dosing rate
2) Patients under observation of one of the physicians who enrolled in the study
3) Patients with GFR under 50 ml/minute
Exclusion criteria:
1) Not receiving at least one of the selected drugs
2) Not being observed by one of the enrolled physicians
3) Patients with GFR higher than 50 ml/minute
4) Patients whose weight and height were not accessible
5) Patients who undergo dialysis
