Inclusion Criteria
1) outpatients aged 18 or over
2) willing to participate in the study
3) having at least the following criteria
(a)receiving medicine treatment from more than one physician
(b)changing dosage regimens
(c) taking at least five medicines
(d) having abnormal laboratory caused by a medicine
(e) not complying to treatment regimen
(f) having an adverse drug reaction (ADR)
Exclusion Criteria
cognitive impairment or communicative problems
