Eligibility Criteria
Inclusion Criteria
- •The inclusive criteria will be People living with diabetes aged more than 18 years,who are known to have type 2 diabetes for more than one year,who have consulted GP and on medications for diabetes,who are residing in the study area,who are willing to participate in the study, whose HbA1c is greater than 7% (HbA1c less than 7% is the current standard established by the American Diabetes Association for acceptable glycemic control; ADA, 2008\) and fasting Blood glucose level \>110mg/dl and random blood glucose level \>200mg/dl(WHO criteria),who have access to cell phone and able to understand and speak Hindi.
Exclusion Criteria
- •the exclusive criteria includes People living with diabetes who have one or more comorbidities and are seriously ill such as MI, CKD, stroke, Unable to give information, whose HbA1c more than 10 %.
