- •Intervention group: patients with cardiovascular diseases who participate in the program and require actual treatment with a participating cardiologist.
- •Control group: patients with cardiovascular diseases who do not participate in the program and require actual treatment with a (non-participating) cardiologist in regular healthcare.
- •Observation period: Patient data from the period 1.1.2014 to 31.12.2016 are used for the retrospective analysis.
排除标准
- •not continuously insured with the SHI throughout 2014-2016 (except deaths in 2015-2016)
- •younger than 18 years at 01/01/2015
- •participating in a different innovative healthcare program (in 2014-2016)
- •patient living outside of Baden-Wurttemberg according to postal code
- •cardiologist’s practice outside Baden-Wurttemberg according to postal code (in 2014)
