Inclusion Criteria
1) min age 18 years
2) pain since at least for 6 weeks OR recurrent pain despite specialized treatment
3) limitation of daily life and percieved health related quality of life because of pain
4) risk for chronicity of pain
5) actual sick leave of at least 4 weeks OR cumulative sick leave of at least 6 weeks during the past 12 months
6) pain relevant diagnosies regarding localization and other conditions
7) pending decision about surgery procedures
8) extensive medical utilization of health care system
Exclusion Criteria
1) cancer of any kind
2) relevant psychological or psychiatric disorder
3) already existing history of chronic pain in the past/actual

- Primary Outcome Measures
Name Time Method pain intensity (subscale characteristic pain intensity, grading chronic pain stages, von Korff), pain related limiation of function (subscale disability, grading chronic pain stages, von Korff), treatment effectiveness (global change item of the German Pain Questionnaire, Satisfaction with therapy questionnaire, ZUF8) from patient perspective are primary endpoints. They will be assessed at admission (except treatment effectiveness), 3 and 6 months follow up via self report questionnaire.
- Secondary Outcome Measures
Name Time Method psychological distress, sick leave (days), health related quality of life, early retirement, direct costs (health policy) will be secondary endpoints. They will be assessed at admission and 3 resp. 6 months follow up via questionnaire, except early retirement and direct costs will be estimated via secondary analyses by the involved cost payer (BARMER).
