•a) These patients shall have to be on optimal standard mamagement of COPD and existing co-morbid conditions.(The management of COPD should include standard pharmacotherapy as per guideline, logistically best possible education and training for self managed rehabilitation and home oxygen therapy as and when indicated).
•b) A proper written informed consent will be taken before including the patients in the study.

•a) Patients unwilling to give proper written informed consent.
•b) Patients with severe co-morbid states affecting the quality of life.
•c) Very advanced COPD with inability to follow up or perform spirometry.
•d) History of exacerbation in the preceding six weeks.
•e) Patients with suspected OSA, ILD, left heart disease and pulmonary embolism (radiological suspicion for ILD, clinical suspicion of OSA from snoring, high BMI, excessive day time somnolence, observed apneas, and / or other features suggesting SDB).
•f) Patients with clinico-radio-echocardiographic suggestions of pulmonary embolism and echocardiography showing the presence of any significant left heart disease.
•g) Patients with known intolerance of sildenafil
