Inclusion Criteria
- •GP patients aged 18 years and older with a frailty score of six or less.
Exclusion Criteria
- •not able to give consent
- •Ill with limited life expectancy with a frailty score of seven to nine
- •Permanent residence in a care facility
- •insufficient language skills for participation
