Inclusion Criteria
patients ASA I-II;aged 18-70 years old;come for ortopaedic surgery distal to elbow; eligible for Bier block.Exclusion criteria:raynod syndrome;sickle cell anemia;allergy to lidocaine-metoclopramide-ketorolac;history of renal disease-liver disease-gasterointestinal bleeding-Parkinson disease;history of extrapyramidal side effect to metoclopramide;administration of NSAID drugs last 24 hours;any neuronal defect in limb under surgery;addiction to opioids.

Name Time Method Pain management. Timepoint: 2; 4; 6; 12; 24 hours after surgery. Method of measurement: visual analogue scale.;Beginning of analgesia. Timepoint: After uses of anesthetic drug. Method of measurement: evaluation patients response to pin.;Dosage of analgesic drug after surgery. Timepoint: First 24 hours after surgery. Method of measurement: nursing report.
