- •ï?§Any patient diagnosed to have heart failure and attending the OPD or admitted to General Medicine and Cardiology Wards (as defined by ACC/AHA 2005 guidelines for the diagnosis and management of heart failure in adults â?? clinical syndrome that impairs the ability of the ventricle to fill with or eject blood
