入选标准
- •Stroke survivors aged 18 years or above, community dwelling, have a modified Rankin Scale score ?3 (moderate to severe disability), and a Montreal Cognitive Assessment (MoCA) score >20.
排除标准
- •Severe dysphasia or mental illnesses.

Two groups
Four home visits and five phone coaching for Eight weeks, once a week. versus Usual stroke rehabilitation care Depends on the participants' clinical needs
