1. End-stage renal disease (ESRD) patients who have undergone hemodialysis for a minimum of 3 months.
2. Serum phosphate level greater than or equal to 5.5 mg/dL or an average serum phosphate level greater than or equal to 5.5 mg/dL over 3 months.
3. Adequate hemodialysis, determined by a Kt/V value greater than or equal to 1.8 for patients undergoing hemodialysis twice a week and a Kt/V value greater than or equal to 1.2 for patients undergoing hemodialysis three times a week.
4. No history of parathyroidectomy.
5. Ability to use Line application or other communication tools independently or with assistance to facilitate communication throughout the study.
6. No malnutrition, as assessed by the Malnutrition Inflammation Score (MIS).
7. Proficiency in the Thai language, including reading, writing, and comprehension.
8. Willingness and ability to actively participate in the study throughout its duration.
1.Change of address/relocation or move of domicile
2.Require hospitalization as a patient due to prolonged illness and unable to participate in program activities.
