There were included Heart Failure patients and their social referee.
For patiets the inclusion criteria were: Age over 18 years old and inferior to 85 years old; Being in clinical follow-up for at least 6 months.
For social referee: Wife/ husbund, daughters/ sons, parents, house workers or any other person who was reponsible to prepare the patients meal at home; Age over 18 years old and inferior to 85 years old; Available to go with patient to the health clinic in all phases of the study.
Patients were excluded: With diagnosis of Hert Failure functional class IV; With recently history of being hospitalized (less than 01 month) before the study; With no condition of effective verbal communication; With diagnosis of mental disorders (described in medical report); Whose social referee who prepares meals at home could not be present at all consultations.
For socials referee: With no condition of effective verbal communication; Wiht diagnosis of mental disorders (described in medical report); Who did not agree in participating of the study.

Name Time Method End-point: The patients who received the intervention showed a significant decrease on salt intake by self-reported methods. <br>The results are:<br>T0:Individual intervention group (mean) = 12.2g salt/day; Collaborative intervention group (mean) = 11.4g salt/day; Individual control group (mean) = 9.8g salt/day; Collaborative control group = 14.9g salt/day.<br>T3:Individual intervention group (mean) = 5.0g salt/day; Collaborative intervention group (mean) = 4.8g salt/day; Individual control group (mean) = 10.4g salt/day; Collaborative control group = 14.2g salt/day.<br>

Name Time Method Secondary end-point: the patients who received the intervention showed a significant decrease on habit.<br>The results are:<br>T0: Individual intervention group (mean) = 3.0; Collaborative intervention group (mean) = 4.0; Individual control group (mean) = 4.0; Collaborative control group = 3.6.<br>T3: Individual intervention group (mean) = 1.2; Collaborative intervention group (mean) = 1.2; Individual control group (mean) = 4.1; Collaborative control group = 4.1.<br>;Secondary end-point: the patients who received the intervention showed a significant increase in self-efficacy.<br>The results are:<br>T0: Individual intervention group (mean) = 4.1; Collaborative intervention group (mean) = 4.3; Individual control group (mean) = 3.4; Collaborative control group = 3.5.<br>T3:Individual intervention group (mean) = 5.0; Collaborative intervention group (mean) = 4.9; Individual control group (mean) = 3.2; Collaborative control group = 2.7.<br>
