• Patients (m/f/d) with acute ACL primary rupture with indication for surgical reconstruction
• Planned ACL replacement surgery with autologous hamstring tendon or quadriceps tendon graft at Sana Dreifaltigkeitskrankenhaus Cologne, Germany
• Patients are capable of giving consent, have full legal capacity and understand the nature, scope and significance of participation in the study
• Concomitant injuries with an intrinsic indication for surgery, that lead to a change in the treatment regimen
• Surgery or injury to the knee joint in the last 5 years (both on the injured leg and on the contralateral leg)
• Comorbidities that are counterindicated to an active exercise program (e.g. severe cardiovascular disease, neurological disease, ...).
• Rheumatic underlying diseases
• Pregnancy

- Primary Outcome Measures
Name Time Method Subjective knee function as assessed by the Knee Injury and Osteoarthritis Outcome Score (KOOS- total score), (1-7 days before reconstruction, on operative day (post-op), and 30, 60, 90, and 180 days post-op)
- Secondary Outcome Measures
Name Time Method • KOOS subscores (PAIN; SYMPTOMS; SPORT; ADL; QOL)
• Mobility (range of motion in angular degrees, active and passive in flexion and extension of the knee joint, measured by means of a goniometer - by the test performers)
• Control/ neuromuscular control (active stretch elevation)
• Strength (quadriceps, hamstrings, plantarflexors; active heel lift, testing on isokinet)
• functional test results (Y-Balance test and Counter Movement Jump incl. movement quality)
• Activity level (Tegner score)
• Ability to cope with everyday life and work
• Return to sport Injury Scale (ACL-RSI)
