- permanent intake of = 5 active pharmaceutical substances with systemic effects (combinations of active ingredients in phytotherapeutics and homeopathics are counted as a single active substance)
- written informed consent
- completed software-assisted medication review
- For patients with more than one completed medication review in the recruitment period, only the first analysis will be considered.
- The inclusion of patients per pharmacy is limited to a maximum of 15% of the calculated samplze size (n = 148), which corresponds to 22 patients per pharmacy.
