Eligibility Criteria
Inclusion Criteria
- •age \= 65 years
- •\- permanent intake of \= 5 active pharmaceutical substances with systemic effects (combinations of active ingredients in phytotherapeutics and homeopathics are counted as a single active substance)
- •\- written informed consent
- •\- completed software\-assisted medication review
Exclusion Criteria
- •\- For patients with more than one completed medication review in the recruitment period, only the first analysis will be considered.
- •\- The inclusion of patients per pharmacy is limited to a maximum of 15% of the calculated samplze size (n \= 148\), which corresponds to 22 patients per pharmacy.
