1. Patients with advanced cancer (Any cancer).
2. Newly referred to Palliative Medicine outpatient department
3. Patient over the age of 18 years and able to provide informed consent, or, through
surrogates, if the patient is sick and not being able to provide consent.
4. The geographical area in greater Mumbai where regular home care services are
not provided: Mira, Bhayandar, Uttan, Dongri, Kashimira, Kasarvadavali, and parts of
Thane (West).
1. Patients who do not agree with home-based care
2. Patients who do not have access to any local general practitioner
3.Patients for whom reliable contact number or method of communication are not
available.
