Inclusion Criteria

Diagnosis of acute cerebral ischemic disease (acute ischemic stroke (ICD-10: I63) or transient ischemic attack (ICD-10: G45), diagnosis of at least one cerebrovascular risk factor, written informed consent to participate in the study.

Exclusion Criteria

Cognitive or physical inability to understand one's own health condition or to independently manage and execute secondary preventive measures after the acute ischemic disease, relevant language barrier, guardianship.
