Brief Summary

The results showed that after the experiment, the experimental group had significantly higher mean scores in terms of knowledge, perceived benefits, perceived severity, self-efficacy, and behaviors to slow the progression of kidney disease than the control group . Further, the experimental group had significantly better clinical outcomes (SBP, DBP, eGFR, BUN, creatinine, albuminuria, FBS, and HbA1c) than the control group.

Inclusion Criteria

1. patients with diabetes and hypertension with CKD 3 complication
2. aged 40 to70 years,
3. blood pressure > 140/90 mmHg at least once in the last six months
4. HbA1C > 7% in the last six months
5. able to communicate, read, and write in Thai
6. lived with their family
7. no complications or severe illnesses during the study that might have caused any obstacle to activity participation.

Exclusion Criteria

1.Need to be relocated away from Samutprakarn province during the study.
2.Request to be withdrawn from the study.
