入选标准
- •Participants will be eligible for the study if ALL of the following inclusion criteria are met:
- •Aged 18 years and over
- •Able to provide informed consent in English or in preferred language of Mandarin or Russian with an interpreter present
- •Diagnosis of coronary heart disease (CHD) (angina and all forms of acute coronary syndrome)
- •Lives independently at home
- •Has access to a device which can connect to the HeartPath+ website
- •Willing to access the HeartPath+ website (if in the intervention arm) and complete study requirements
排除标准
- •Participants be ineligible for the study if ANY of the following exclusion criteria apply:
- •Unable to provide consent
- •Living in supported accommodation or aged care
- •Clinically unstable/unwell (e.g currently in ICU)
- •Terminal diagnosis
- •Participant not willing to use the HeartPath+ website or is unable to access the site
