Inclusion Criteria
- Elderly, 65 years of age and older
- Community-dwelling
- Referral from the general practitioner due to chronic condition
- FAC 3 or more
Exclusion Criteria
If an older adult is insufficiently instructible after assessment by the physical therapist, he or she can not participate in this study.

- Primary Outcome Measures
Name Time Method <p>Fear of falling will be mapped with the Falls efficacy scale international<br /><br>(FES-I). Physical activity will be measured by an accelerometer. The Fitbit<br /><br>Classic will be used in this study. Seen the limitations of the Fitbit Classic<br /><br>(it is not suitable in water), physical activity will also be measured through<br /><br>the Lasa Physical Activity Questionnaire, in short LAPAQ. The physical capacity<br /><br>will be measured through the Short Physical Performance Battery, in short SPPB.<br /><br>Balance will be measured through the Timed up and go test, in short TUGT.<br /><br>Muscle strength of the lower extremities will be measured through the Timed<br /><br>Chair Stand Test, in short TCST. Endurance will be measured through the<br /><br>2-minute step test, in short 2MST. Flexibility will be measured trough the<br /><br>Chair Sit and Reach Test, in short SRT.</p><br>
