Inclusion Criteria
patients who come to endoscopic examination at Thammasat University hospital
Exclusion Criteria
1. previous gastrointestinal surgery
2. CA esophagus
3. CA stomach
4. Upper GI bleeding
5. history of corrosive ingestion
6. pregnancy
7. allergy to lidocaine
8. allergy to simethicone, N-acetyl cysteine, peppermint, sodium bicarbonate
9. emergency endoscopy
