1. All adult patients ( >18 years) who will receive invasive mechanical ventilation through endotracheal tube/tracheostomy tube for hypoxemic respiratory failure.
1. Age less than 18years.
2. If patient received MV >48 hours in last 6 months.
3. Patients will not be re-recruited.
4. Duration of MV < 3 days.
5. Profound neurological deficit leading to sleep apnea.
•Psychiatric, agitated or non-cooperative patients.
•Patients with ascites.
•Post – thoracotomy patients with suspected phrenic nerve palsy.
•Those not giving consent.
