Inclusion Criteria

1.) Adults with clinical manifest coronary heart disease (d.h. condition after stabile of unstable Angina pectoris, acute coronary Syndrome, percutaneous coronary Intervention and after Bypass Operation). Approx. 50% of the total Patient Population must be female. Approx. 30% must have a left ventricular rejection volume of = 40% and have therefore a cardiac insufficiency)
2.) relatives of these patients
3.) German as mother tongue or have adequate language skills incl. the ability to read
4.) min. 18 years old
5.) written informed consent

Exclusion Criteria

1.) under 18 years
2.) insufficient German language knowledge
3.) high instabile physical and psychological Status(e.g.) severe heart insufficience, instable Angina pectoris, other severe physical illness, acute suizidality, delirious Syndrome, severe dementia)

Primary Outcome Measures

Analysis of the uality of care of patients, who suffer from a coronary heart disease and a comorbid psychological disease. Detection of barriers in guideline based care, understanding of backgrounds. In the Population of the patients and their relatives a Special Focus shall lay on gender and age. On the side of the health care providers organization specific characteristics shall be captured. In the results we try to find hints with whom we can change the care to a more Patient centeredness care. This shall be the first step to a value-based care for patients with somatic-psychological comorbidity. <br><br>
