Inclusion Criteria
1.Outpatients and in patients with histopathology evidence of head and neck cancers receiving definitive intent radiotherapy/ concurrent chemo radiotherapy
2.Age > 18 years
3.Consenting to participate in the study
Exclusion Criteria
1.Patients planned for palliative intent radiotherapy
2.Patient previously seen by Department of Palliative Medicine and Supportive care
3.Prior history of radiotherapy

- Primary Outcome Measures
Name Time Method Treatment related skin and mucosal toxicity will be recorded using CTCAE grading and the change in symptoms will be recorded using FACT HN and ESAS revised questionnaire.Timepoint: Patients will be reassessed on weekly basis up to 4weeks and every 2 weeks as part of routine follow up assessment until planned schedule of radiotherapy completed to identify change in symptom burden after supportive care intervention
