•Individuals who regularly attend the Department of Neurology at Dokkyo Medical University Nikko Medical Center, or the Miyori Clinic in Nikko City where the same neurologists provide outpatient care.
•Adults aged 65 years or older at the time of enrollment.
•Individuals clinically diagnosed with Mild Cognitive Impairment (MCI) or mild dementia by a neurologist.
•Mild Cognitive Impairment (MCI) is defined as a condition in which activities of daily living (ADL) are generally independent, but objective cognitive decline is observed through neuropsychological assessment, without meeting the diagnostic criteria for dementia.
•Individuals who receive sufficient explanation about the study from the principal investigator or co-investigators and provide written informed consent.

•Individuals who are clinically judged to have moderate or more advanced dementia and are considered unable to complete the Egao Note or study assessments
•Individuals with severe physical disability visual impairment motor dysfunction or significant difficulty in writing or reading that makes it difficult to complete study procedures
•Individuals with severe psychiatric disorders such as schizophrenia or major depressive disorder and are considered unable to participate in the study
•Individuals who are unlikely to maintain regular outpatient visits during the 12 week intervention period and the pre and post assessments
•Individuals who are judged by the principal investigator or co investigators to be unsuitable for study participation
