Inclusion Criteria

1.tender at base of thumb, 2.Grind test, Axial compression test : positive, 3.A Robert view of thumb:joint narrowing, joint widening , osteophyte, 4.Eaton and Littler classification 1-4, 5.no history of steroid injection, 6.failure of medication

Exclusion Criteria

1.History of steroid injection at first carpometacarpal joint before, 2.other pain disease at base 1st metacarpal joint, 3.allergy to steroid or xylocaine ,4.severe infection within 4 weeks , 5.poor control diabetic disease, 6.patient had been life vaccine injection within 2 weeks
