Inclusion Criteria

congestive heart failure (NYHA 3-4), dementia (GDS =4), or chronic obstructive pulmonary disease (Gold 3-4, group D), at least one consultation with the GP in the last three months, no support from SPHC services, no tumour diagnosis, signed consent form

Exclusion Criteria

congestive heart failure (NYHA <3, no hospitalisation in the last 12 months), dementia (GDS <4), or chronic obstructive pulmonary disease (Gold <3), no visit at the GP in the last three months, a current tumour diagnosis, current support from specialized ambulatory palliative care, no signed consent form
